Between 8 and 17 July 2021, South Africa experienced unprecedented violent unrest in KwaZulu-Natal and Gauteng: an estimated 354 deaths and more than R50-billion in economic damage. The SAHRC convened an Imbizo on 23 July 2021 and then launched a National Investigative Hearing into the unrest. In November-December 2021 the hearing heard from, among others, National Commissioner Gen Khehla Sitole and KZN Commissioner Lt-Gen Nhlanhla Mkhwanazi — whose combative testimony ('there is something wrong with society') was an early preview of his later confrontation with political leadership. The final report, 'July's People', was published on 29 January 2024, and in 2026 the Commission released an update report tracking implementation of its recommendations.

At the SAHRC's National Investigative Hearing into the July unrest, National Commissioner Gen Khehla Sitole and KZN Commissioner Lt-Gen Nhlanhla Mkhwanazi testify in November-December 2021. Mkhwanazi's evidence is combative: he disputes former Defence Minister Nosiviwe Mapisa-Nqakula's account as 'pure fabrication' and tells the panel 'there is something wrong with society'.
